4/14/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Jahsim. Upon arrival to the home, Jahsim appeared to be in a positive mood. Ms. Shabazz informed QMHP-C, she had an issue with Jahsim stealing from her and he continued to deny when other siblings told her that he did. 2. Behavior: Jahsim participated in the session and needed redirection due to Jahsim becoming upset with the fact he did not steal from his mother and his siblings lie all the time so what is the use of her listening to them. 3. Behavior: QMHP-C facilitated a discussion with Jahsim focusing on session held thus far. QMHP-C encouraged Jahsim to be open during discussion. QMHP-C identified areas of improvement observed over the period of counseling sessions and what continues to need improvement. QMHP-C engaged Jahsim in a therapeutic activity to assist him with opening up and discussing their feelings honestly. 1.Intervention: QMHP-C began session by processing with Jahsim his current emotional state. QMHP-C reviewed interventions from the previous session and explored with Jahsim his behaviors while in the community and with his family and peers. QMHP-C encouraged Jahsim to utilize positive social skills to include refraining from physical and verbal aggression with others and recognizing boundaries. 2.Intervention: QMHP-C explored with Jahsim any current instances of stealing and encouraged Jahsim to respect the property of others. QMHP-C engaged Jahsim in a therapeutic discussion and encouraged Jahsim to verbalize the triggers to his inappropriate behaviors. QMHP-C ended session by reviewing interventions with Jahsim and his mother and encouraging them to provide feedback on the session. 3. Intervention: QMHP-C encouraged Jahsim engage in positive interactions with QMHP-C to assist him with being able to control is anger when triggered in the home, school and community setting. QMHP-C assisted Jahsim with identifying the cause of these feelings and more appropriate ways to resolve these conflicts. 1.Response: Jahsim was receptive to QMHP-Csâ interventions and services provided during the session. While processing with Jahsim his current emotional state, Jahsim shared that he was in an irritated mood. While exploring Jahsimâs behaviors in the community, Jahsim shared that he enjoys being in the community and meeting new people. Jahsim shared that he does not engage in physical altercations outside of altercations with his siblings. 2.Response: While exploring stealing, Jahsim denied any stealing behaviors. Jahsim actively participated in the therapeutic game and shared that he wants people to recognize positives in him instead of negatives and that this was one of his triggers.
